My favorite hammer head flew off mid-swing last Tuesday

I was finishing up a leaf hook order and on the third heat, the head just launched off the handle and bounced off the shop wall. Turns out the wedge had worked loose over a few months and I never checked it, my fault completely. I spent 20 minutes digging the wedge out of a bucket of old oil to reset it with a new one I had lying around. Has anyone else had a handle fail on them out of nowhere, and do you re-wedge heads or just retire them?

The whole "my fault completely" part really hit home because it's always the small stuff you skip checking that bites you. I've noticed this pattern everywhere, not just with tools, like how people never replace their car tire pressure monitors until the light's been on for months. Better to spend the five minutes resetting a wedge than the twenty digging it out of a bucket of oil, that's for sure.
