I finally had enough of my garage floor cracking around the car lift bolts

About 6 months ago I installed a two-post lift in my detached garage here in Phoenix. Thought I did everything right with the concrete anchors and torque specs. Well last week I noticed these hairline cracks forming right around three of the base plate bolts. Then yesterday one of them actually chipped out a chunk of concrete the size of a golf ball. I freaked out and immediately lowered the car down. Now I'm stuck with a lift I can't use and a floor that needs patching. I'm thinking about cutting out a 4x4 foot section and pouring new reinforced concrete around each post base. Has anyone else dealt with concrete failure around a lift anchor? What did you end up doing to fix it long term?

Did you figure out how thick the original slab was before you poured it?

Slab was 3.5 inches in most spots but closer to 2.5 in a few places near the edges. Pretty common for old pours to be uneven like that. You had a similar issue with yours right?

Totally feel your pain lol, it's wild how much these old slabs can vary across the same pour. Mine was pretty similar, like an inch difference in some spots made leveling a headache.
