My crawler crane's main cable keeps slipping off during light lifts

I've run the same machine for close to ten years now, and this just started last month. When I'm bringing up empty hooks or light rigging, the cable pops off the top sheave every few cycles. I checked the groove for damage and made sure the tension was right, but it still happens. Has anyone dealt with this kind of thing before? I'm at a loss.

Ever notice how stuff waits until you're totally comfortable to act up? Your crane running fine for ten years then acting weird is the perfect example, like that pickup starter story. Machines just love to mess with us lol.

Honestly, that sounds like the machine version of my back going out when I bend over to pick up a feather. Tbh, after ten years I'd be looking real hard at that top sheave again, maybe the groove is just worn slick in one spot you can't easily see. Ngl, I'd probably be crawling all over that thing with a flashlight and a straight edge feeling like a fool.
