Warning: I ran a rigging check wrong for years and didn't know it

I always eyeballed the sling angle on my picks until last month when a safety guy from a refinery pulled out a tape measure and showed me I was off by almost 20 degrees on a 4-leg bridle. That's way over the rated load limit and I had been doing it that way for probably 5 years. Has anyone else had a basic habit turn out to be unsafe?

Had a buddy who ran his forklift forks too low for years until he caught them on a dock plate and mangled it.

Funny how it takes one big bang to undo years of bad habits, but I've seen guys run them like that for a decade with no issues. @mark_carr7, was your buddy's dock plate actually damaged that bad or did it just scare him straight? Seems like one of those things where the risk is real but the payoff is getting the job done faster.
