Lost a $12k retainer after a junior designer sent the wrong attachment

Was on a call with a client in Austin last Thursday and they forwarded me the messed up file another agency's junior sent... Has anyone else had to put strict approval layers in place after one stupid mistake cost you big?

Made it a rule that nothing goes out the door without a second pair of eyes on it. Period. After my shop assistant sent the wrong grooming instructions to a client and they showed up with a poodle cut for their golden retriever, I lost half that client's yearly board plus the referral business. Now we do a simple two-step check. The person who preps the file initials it, then someone else reviews and initials before it gets sent. Takes maybe 30 seconds extra per job but saves everyone a lot of headaches. Also put in a 15 minute cooling off period for anything with attachments or money involved. You'd be surprised how many typos and wrong files get caught just by stepping away for a bit.

Oh wow, I used to be totally against the whole double-check thing. I thought it was just a waste of time and that people should be trusted to do their jobs right. But hearing about that golden retriever mix-up, holy cow, that's brutal. Your 15 minute cooling off period is actually genius though. I bet it catches so many little things people would miss when they're in a hurry. Definitely makes me rethink my whole stance on this.
