Lost a $4k retainer client last Tuesday because I forgot to update a single tracking pixel

Had the worst Wednesday of my career when their site analytics showed zero conversions for two weeks and the client found it before I did, has anyone else had a tiny mistake snowball into losing a big account?

Remembered reading a story similar to what @karenh56 mentioned about the comma thing. There was this whole thread in a marketing group I'm in where someone lost a $10k contract because they used the wrong link in an email blast. Client opened it, saw it went to a dead page, and just pulled the whole campaign. It's wild how one tiny slip up can make clients lose all trust in you. Pixel tracking is one of those things where clients expect perfection since it's literally just copying and pasting code.
